Reminder: Insurance Waiver Forms and Fees due by end of January

A final reminder that the mandatory annual insurance premium is due at the end of January (i.e. by next week), and all students who plan on attending classes at the Kuan in 2011 are requested to fill out an updated Application & Release Form. Also, a $20 one-time annual fee is requested to help defray the cost of the premium.

Below is a list of the folks from whom forms have been received. If your name is not on this list, and you plan on attending classes in 2011, please fill out and submit the form and fee.
